CALL A PCP

GENERAL CONSENT TO TREAT

Effective Date: August 8, 2026

This consent authorizes Call A PCP to provide reasonable and medically appropriate healthcare services. By signing below, you acknowledge and agree that:

  • You voluntarily consent to receive medical evaluation, diagnosis, and treatment from Call A PCP through telemedicine.

  • This consent applies to services provided by Abigail Soto, MD, and any authorized healthcare provider working on behalf of Call A PCP, if applicable.

  • This consent remains in effect for future treatment provided by Call A PCP unless you revoke it in writing.

  • You have the right to discuss your diagnosis, treatment options, expected benefits, potential risks, and alternatives with your healthcare provider before making healthcare decisions.

  • You have the right to ask questions about your care, and all questions will be answered to your satisfaction.

  • You may refuse or discontinue treatment at any time. Refusing treatment may affect your provider's ability to diagnose or manage your condition.

  • If your provider determines that your condition requires in-person evaluation, emergency care, or specialist care, you understand that you may be referred accordingly.

  • I understand that laboratory testing alone does not establish an ongoing physician-patient relationship or replace comprehensive medical care with my primary care provider. I understand that abnormal or concerning results may require additional evaluation, follow-up, referral, or an in-person examination.

Patient Statement of Consent

"I voluntarily request that the healthcare provider(s) at Call A PCP provide reasonable and medically appropriate evaluation, diagnosis, treatment, and follow-up care for the condition(s) for which I am seeking medical services. I understand that treatment recommendations may include laboratory testing, diagnostic imaging, referrals, or prescription medications when medically appropriate.

I understand that Call A PCP provides non-emergency telemedicine services only. If I believe I am experiencing a medical emergency, I will call 911 or go to the nearest emergency department.

I understand that prescriptions are issued only when medically appropriate and that Call A PCP does not prescribe controlled substances through its telemedicine practice.

I certify that I have read and understand this Consent to Treat. I have had the opportunity to ask questions, and I voluntarily consent to receive medical care from Call A PCP."
