Each request is evaluated individually. Medication approval and the quantity or number of refills prescribed are based on the physician's clinical judgment and your individual medical history

• Medication name

• Current dose

• Photo of your prescription bottle or medication label

• Your preferred pharmacy

• Recent laboratory results, if applicable

• Information from your previous prescribing physician, if available
